Health Screening Checklist (effective October 6, 2022)
The SCCC Board highly recommends for curlers to complete the Health Screening Checklist every week prior to arriving at the curling center. If you answer YES to any of the 3 questions below, please DO NOT come to the SCCC to curl on that particular week.
1. Do you have any new-onset (or worsening) of any of the following symptoms:
Fever or feeling feverish
New Cough
Shortness of breath/difficulty breathing
Runny nose
New Sore throat
New Muscle Aches
New Headache
New Loss of smell or taste
2. Have you had close contact with a confirmed case of COVID-19 in the last 14 days?
3. Have you had close contact with an individual who has any one of the first 5 symptoms on the list (above) AND who is a close contact of a confirmed case of COVID-19 in the last 14 days?
